When Dean called Martha Baby, I was like did he really just say that only for her say it in the next line. Brilliant! The prelude speak to the kiss was exactly how I imagine Dean would have done it.

Poor Martha, I would be scared as hell with every demon, vamp, and whatever else out there trying to get me. I would probably have said "Fuck it, I'm done," took Dean's hand walked back to the house, and fucked just for the hell of it.

I wanted to slap Ellen so bad!!! She knows how Dean feels about his brother and Father, and then to throw it in his face like that was just mean. SKANK! (In case you couldn't tell, I can't stand anyone trying to go off on my Dean!!!!!!!)

So now the Doctor knows Dean's secret. Maybe now they can figure out how the switch occurred.
